Geriatric Care Consultants LLC is a Los Angeles based private geriatric care management practice designed to guide seniors and their family members through all aspects of the aging process and even the most difficult decisions related to care. No two individuals are alike. For this reason our services are tailored for your specific situation, and we take great care in understanding your needs.  Our services range from single assessments to long term care plans and ongoing Care Management.

Our Assessments (1-2 hour process in the home/facility) include the following:

  • Assessment of functional ability
  • Home safety
  • Memory loss
  • Medical status/need
  • Legal issues
  • Financial issues
  • Psychological issues

A comprehensive care plan is then created to meet individual goals and need. The whole process is 5-6 hours. Families often benefit from a  thorough care plan which helps guide the family in the right direction. Once a care plan is created we work together to facilitate the next step in your loved ones care.

  • Hospital crisis management/ discharge plan
  • Coordination of care
  • Crisis Intervention
  • 24 hour availability
  • Patient Advocacy and Education
  • Bill paying
  • Provide telephone and/or email updates to family members
  • Hiring Caregivers
  • Oversight of caregiver quality and compatibility
  • Attendance at Physician appointments
  • Placement Services
  • Counseling and Support (in person, by phone)
  • Caregiver education and support
  • Liaison between family members and local professionals
  • Referrals to & Coordination with Local Professionals (medical, legal, financial)
